The two key MEPSO activities are electricity transmission and power system control. MEPSO also organizes and dispatches the transit of electricity through Macedonia and takes care of the power system balancing.

To perform its activity AD MEPSO has been granted three licenses by the Energy Regulatory Commission with a validity time of 35 years until 2040. The first license relates to doing energy activity of electricity transmission. The second license is for doing energy activity in terms of organizing and managing the electricity market and the third license is doing the energy activity in terms of power system control.

For its business AD MEPSO charges an electricity transmission fee set by the Energy Regulatory Commission (ERC) based on the AD MEPSO expenses. This means that AD MEPSO is not a profitably-oriented company but it provides services to citizens and Macedonian economy.

MEPSO has in its ownership the high voltage transmission grid, substations for conversion of voltage from 400 to 110 kV.The transmission lines of 2.092 km in total length, substations with 148 transformers of 6.417 (MVA) installed power are the ownership of MEPSO. The first subsidiary is the Operator of Electric Power System and its task is to control the system and provide a harmonized relation between the consumers and electricity generators (local generation and import).

The second subsidiary is the Operator of Transmission Grid and its task is to take care of the transmission lines, substations and transformers and other installations to be in sound operating condition enabling regular and duly electricity flow.
